Checking In A Patient

In order to Check-In the Patient, you will need to access the SportGait Medical Application on your assigned Microsoft Surface Pro. **Please use the SportGait Reference Sheet for instructions on how to log in to the Surface.**

HOW TO ACCESS THE SPORTGAIT MEDICAL APP:

Your login to the SportGait Medical App will be your work email; a temporary password is emailed to you.

* If you know that you have been given a SportGait Medical App account but have not received a temporary password, check your spam or junk folder.

HOW TO CHECK IN A PATIENT:

You will be given 2 options upon logging in: “Find a Patient” or “Add New Patient”.

 

To Find A Patient:

Select “Find a Patient”. You will be able to search for the patient by their name, SportGait ID, or Date of Birth on the “Patients” page.

If the patient in question has a file already in the system, you will select “View”.

In the “View” screen you will be able to see whether or not the patient has already completed a Brain Physical. Here you can select “Check-In” to start the Brain Physical.

You will be asked to select the purpose of the visit: Baseline, First Procedural, or Follow Up Evaluation.  We will cover when to select each type of visit later in the course.

 

To Add a New Patient

To add a new patient, you will select “Add New Patient“.

You will be asked if the patient has a SportGait ID.

If they say “no” or they aren’t sure, make sure to ask the patient if they’ve performed a SportGait Brain PhysicalTM in the past (at any location) or if they have an account in the SportGait Mobile account.  Answering “yes” to either of these questions means they DO have a SportGait ID.

IF THE PATIENT DOESN’T KNOW WHERE TO FIND THEIR SPORTGAIT ID:
The best way to locate the ID, is by opening up the SportGait Mobile App on the Parents/Athletes phone.  Ask the patient to select the appropriate athlete’s profile, then select the “Doctor” tab.  They will then need to select “Share health history”.  The SportGait ID will be located on this screen.

If they DO NOT have one, you can add them to the system by selecting “Add New Patient”. Enter in the patient’s information to generate a patient file to check-in.

PLEASE NOTE the “Patient ID” Field – this is to add the Patient’s assigned EMR number at your facility.  SportGait DOES NOT connect to your EMR. The field is only for reference in looking up patients.

Once the new patient has generated, make sure to “View” the patient file first, this will allow you to see if a SportGait Brain Physical™ has recently been administered to the patient in the past at a different location.

By default, the Patient Activity will only show information for the last two weeks.

Select “Check-In”.

You will be asked to select the purpose of the visit: Baseline, First Procedural, or Follow Up Evaluation.  We will cover when to select each type of visit later in the course.
